All required health forms for new registrations this school year that have not already been submitted to our health staff can be found online:
Go to HCPSS Forms…scroll down to “Health Forms” to find the Physical Examination, Dental, Health Survey and Lead testing forms.
**Note - the Lead Testing form is required for all new K and 1st grade students only. Once forms are completed, feel free to email or scan to the Health Room staff.
